Bridal COntracT for jessica mccall
The wedding date is July 10th, 2021 and will be held at Zorvino Vineyard in Sandown, NH.
Hair & makeup services will be provided at the location. Arrival time of 11am and end time of services is to be at 4:30pm. There are a total of 6 hair services scheduled as of 01/17/21. The Bride’s hair trial has already been scheduled and paid for. Stylists will arrive at 11:00am for setup to be ready for 11:30am start time with 2.
Terms of Agreement:
This contract is between KO Beauty Bar, LLC/Kayla Ouellette and The Bride (Client).
Deposit - A non-refundable/non-transferable deposit of $200.00 is required at the time of booking to hold the date of your special day and allotted time agreed upon above. The Bride has paid the non-refundable deposit through VENMO. Which covers the travel fee and reservation of Kayla Ouellette and Carrie Hardy.
Card on File - A credit card on file is required. Having a card on file gives the bride one less thing to worry about on the day of. Payment for the bride’s services will be charged to this card along with any last minute cancellations on the day of. Payment via Venmo, check, cash or PayPal is preferred to avoid fees.
Stylists - KO Beauty Bar, LLC will provide 2 stylists to perform services to the Bride and her guests on the wedding day.
Traveling – A travel fee of $200.00 will be charged in addition to services if the services are to be performed at a location other than KO Beauty Bar.
Cancellations - Trial run cancellations made with less than 24 hour notice will be charged to the card on file at a rate of $30/service. (See “Card on File” section regarding day of service cancellations.)
Services – All services collected from bride and guest forms shall be reviewed 2 weeks prior to the wedding and paid for by July 3rd, 2021 (gratuity can be paid for on the day of). Additional fees will be charged for any additional services provided on day of event (extra preparation of hair and/or skin to allow for services, add on flower girl, etc.).
COVID-19 - On the day of the wedding, KO Beauty Bar, LLC reserves the right to take temperatures (with a touch-less thermometer), do a COVID-19 question screening on each person receiving services and require that all personnel in the room (that services are being performed in) to wear a mask and social distance. KO Beauty Bar, LLC will be as flexible and accommodating as possible for any reschedules that are needed due to COVID-19, however we can only accept rescheduled dates per our availability. Any monies paid beyond the non-refundable deposit will be refunded in the event that we are not able to accommodate a rescheduled date. Any cancellations or last minute rescheduled dates need to be confirmed with KO Beauty Bar, LLC at least 48 hours prior to the original wedding date to receive a refund of any payments (only beyond the non-refundable deposit).
